Qualifications for Enrollment

  • Any plans (behavior, life plan, needed medical, etc…) should be submitted to the Day Habilitation Coordinator.

  • Applicant must show a willingness to participate in new or unfamiliar activities.

  • Applicant must be independent in all areas of personal hygiene including toileting.

  • Applicant must be self-medicating in the event that they take prescription or over-the-counter medications.

  • Applicant must have a documented diagnosis of a developmental disability.

  • Applicant must have Medicaid and be enrolled in the waiver.

  • Applicant must be over the age of 18; there is no maximum age limit.

  • Applicant must be willing to navigate community settings safely.

  • Applicant must be free of any behavioral concerns (physical or otherwise) while in program, for at least 12 months prior to intake.

DCOP Enrollment Process

1). Contact your Care Coordinator regarding your interest in our program

2). There will be an initial meet-and- greet style meeting with the interested individual, family/guardian, Care Coordinator and Day Habilitation Coordinator

3). Your Care Coordinator will submit a universal application for enrollment in DCOP

4). A visit with the individual will be set up that will allow them to experience the program first hand

5). If all parties agree that the individual is best served by DCOP, the Day Habilitation Coordinator, and Care Coordinator will complete the enrollment process
