The Service Coordination Department monitors all services
which our consumers receive. The Service Coordinator ensures that
the services meet the consumer's needs and enhance individuality,
integration and independence. |
|
|
Acting on input provided by the consumer and his/her
family member, the Service Coordinator will advocate for services to
stay the same or for there to be a change in services. As a
facilitator of communication, the Service Coordinator ensures that there
is a good flow of information to the consumer and between all service
providers. |
